There are few head-to-head trials assessing the most effective program for weight loss, but one RCT in the United Kingdom, which compared three commercial programs (WW, SW, and RC) with three NHS programs (Size Down, a nurse-led, and a pharmacist-led intervention), reported that all programs resulted in significant weight loss at 12 weeks (1.37–4.43 kg), but only the commercial programs achieved significantly greater weight loss (mean difference 2.3 kg) at 1-year follow-up.32 The results from this RCT suggested that WW was the most effective intervention, and this was supported by a further study of an NHS primary care slimming on referral scheme comparing RC, SW, and WW, which reported that those attending WW lost more weight (mean difference 1.15 kg).38 A more recent trial, designed to assess noninferiority of the three programs commonly used in the United Kingdom (an NHS program, RC, and SW), compared with WW failed to show significant differences at 1-year follow-up and concluded that differences between commercial programs were small and of minor clinical importance.37 The British Broadcasting Corporation “diet trials” also reported significant weight loss over 6 months with no differences between WW, RC, Slim-Fast, and the Atkins diet,48 and a 1-year study in the United States comparing WW, the Ornish diet, the Zone diet, and the Atkins diet stated that there were no significant differences in weight loss between diets.49 In summary, it appears that commercial programs are slightly more effective for weight loss than those offered by the health sector, but there appears to be little or no difference between commercial programs.
